puborectalis and external anal sphincter muscles

چکیده
The innervation of the puborectalis and external anal sphincter muscles was studied in 32 patients with idiopathic (neurogenic) faecal incontinence, 12 of whom also had complete rectal prolapse, using transcutaneous spinal stimulation, transrectal pudendal nerve stimulation, single fibre EMG, anorectal manometry, and measurement of perineal descent. Fourteen normal subjects served as controls. Significant increases in the spinal motor latencies from Li to the puborectalis and external anal sphincter muscles were shown in all 32 incontinent patients (p<0O01). The single fibre (EMG) fibre density was increased in the puborectalis muscle in 60% and in the external anal sphincter in 75% of patients. An increased pudendal nerve terminal motor latency was found in 68% of patients; 69% had an abnormal degree of perineal descent and all had reduced anal canal contraction pressures. These data show that the different innervations of the puborectalis and external anal sphincter muscles are both damaged in patients with anorectal incontinence.

متن کاملExperimental Study No. 1: Directional muscle forces activate anorectal continence and defecation in the female
EMG and radiological data suggest a striated muscle sphincteric mechanism 1 acting during fecal continence control. Though puborectalis and external anal sphincter muscles have been demonstrated to contract during effort,2 the role of the other pelvic floor muscles has not been elucidated. The mechanism of defecation is also poorly understood. According to one description 1 feces enter the anal...
متن کاملThe Musculo-Elastic Theory of anorectal function and dysfunction
The mechanics of defecation and fecal continence are poorly understood. Valvular theories for continence 2 rely on raised intra-abdominal pressure to force the anterior wall of the rectum downwards to close off the anorectal junction. Such theories are not consistent with EMG and radiological data which suggest a striated muscle sphincteric mechanism.3 It has been demonstrated that puborectalis...
متن کاملIs the Puborectalis Muscle Part of the Levator Ani Muscle?
1 on the anatomy of the levator ani, and especially the puborectalis muscle by use of MRI and CT. The authors state that the levator ani has a transverse portion and a vertical portion and that the puborectalis muscle is a u-shaped muscle outside the vertical portion. Based on those findings, the authors conclude that the puborectalis is not a part of the levator ani muscle. The concept that th...

Biopsies of the external anal sphincter, puborectalis, and levator ani muscles have been examined in 24 women and one man with long-standing anorectal incontinence, 18 of whom also had rectal prolapse, and in two men with rectal prolapse alone. In 16 of the women anorectal incontinence was of unknown cause, but in eight there was a history of difficult labour. Similar biopsies were examined in ...
